We are recruiting on behalf our client in Chelmsford for a full time support worker who will assist patients being discharged or returning to hospital for appointments. You will support clients with their daily living and helping to promote their independence. Although no personal care involved you may need to assist with prompting of daily routines.
You will need to be kind, caring, compassionate, chatty and have a down to earth personality.
The ideal candidate would have a full clean driving licence and be provided with a vehicle to transport the patient.
The candidate would need to be able to think on their feet and there may be occasions when you will be working alone.
The ideal candidate will be providing practical and emotional support in the service user's own home and signposting to other services where appropriate to enable the individual to live independently in their own home.
Provide reactive, focused and flexible support to adults
Re-enable service users to remain in their own home and prevent hospital re-admission
Respond to referrals from Clinicians and Community Health Professionals
Assess service users' needs, complete and follow a support plan for the individual
Provide practical and emotional support in the service user's own home and signposting to other services where appropriate
To enable the individual to live independently in their own home
Excellent interpersonal and communication skills
Ability to work as part of a team and on own initiative
Able to plan own workload
Ability to deal with queries in a diplomatic, professional and confidential manner
Good telephone manner
Ability to communicate with health and social care professionals
